Paul Rimar established Patient Care Coordination (“PCC”) as a Service Coordination agency in the fall of 2014. Paul believed that if he recruited the right team and stayed true to his vision, PCC would not only grow, but lead the way for others to follow as a servant of the community and its vulnerable members. PCC began with zero participants in a small Philadelphia office and quickly outgrew the space by mid-2015. PCC then established itself in the central Philadelphia business hub to increase accessibility for the participants it serves. Today, PCC is a leading provider of Service Coordination in 14 counties across Pennsylvania. It also now provides Nursing Home Transition to focus on bringing individuals back into their communities.
In 2019, PCC expanded its business by contracting with 3 major managed care organizations in Pennsylvania. This led to significant financial growth and recognition as one of the top service coordination entities in Pennsylvania. As such, PCC was reorganized to better address managed care organization and participant needs while increasing its headcount over 100%. Patient Care Coordination was recently recognized by Inc. as the 691st Fastest Growing Private Company in America with 3-year growth of 631%.
